Once again, check the document requirements on the website of the embassy/consulate in your country! Documents must not be older than 180 days on the day when you submit the application.

Attention: Keep the original insurance policy with you. All you need to do is attach a copy to your visa application. You will need the original insurance policy when affixing the visa into your passport and for possible use in the Czech Republic.
